Sussex Outreach Support (SOS) helps people in rural communities access the support they need, when and where they need it.
Through a network of local hubs and a mobile outreach unit, we bring services closer to home and make it easier to find the right help without having to navigate complex systems alone.
Our Mission
Our mission is to reduce barriers to support by connecting services and communities more effectively.
We aim to reduce isolation, improve wellbeing, and create more joined-up, accessible pathways to health, advice and support — particularly for those living in rural and hard-to-reach areas.
Our Story
Sussex Outreach Support CIC (SOS) was founded by a GP who recognised both the increasing pressures on the NHS and the difficulties rural communities face in accessing timely support.
SOS was created not to duplicate services, but to connect them — helping people navigate what already exists and ensuring they remain supported throughout their journey.
Working alongside voluntary and statutory partners, SOS delivers support through two key approaches:
- Local satellite hubs in accessible community spaces such as village venues and railway stations, offering one-to-one appointments, drop-in advice, group activities and wellbeing sessions
- A mobile outreach unit, funded through the Levelling Up Partnership, designed to bring services directly into smaller villages and isolated communities where access is limited
This flexible, place-based model allows SOS to reach people who might otherwise struggle to engage due to transport, isolation or digital barriers.
By coordinating services and improving access to early help, SOS reduces pressure on General Practice and helps ensure people receive the right support, in the right place, at the right time.
We don’t replace services — we connect them, coordinate them, and bring them closer to the communities that need them most.

Our Aim
Sussex Outreach Support aims to provide better access to services and activities to all those living in Rural Rother, to help alleviate and prevent poverty, ill health and isolation.
We offer free help for:
- Supporting rural communities
- Increasing access to advice and support services
- Increasing access to health and wellbeing services
- Supporting community activities
- Alleviating isolation
- Providing access of 1-2-1 appointments, drop in advice and group sessions
